Granite VNA is seeking a compassionate Medical Social Worker (MSW) to support patients and families in the home setting. In this role, you’ll help individuals navigate the emotional, social, and practical challenges of illness, recovery, and end-of-life care—making a meaningful impact where it matters most.
Key Responsibilities:
- Assess patient and family needs and develop individualized plans of care
- Provide counseling, education, and support in the home setting
- Coordinate care with interdisciplinary teams and healthcare providers
- Assist with hospice education and referrals when appropriate
- Maintain accurate, timely documentation of care and patient progress
- Master's degree in social work (MSW) from an accredited program
- Minimum of one year of healthcare social work experience preferred, new graduates of MSW programs will be considered
