Provide skilled nursing care in patients' homes, including assessment, care planning, education, and coordination with providers and interdisciplinary teams. Supervise LPNs and LNAs while supporting high quality patient outcomes
Qualifications
- RN license (NH or compact/NLC)
- 1+ year recent acute, med/surg, or skilled nursing experience
- Valid driver's license and reliable transportation
