Granite VNA is seeking an Occupational Therapist to provide in-home care to patients in the Wolfeboro area and surrounding communities. This role offers autonomy, flexibility, and the opportunity to deliver meaningful, one-on-one care that helps patients safely return to daily living.
Key Responsibilities:
- Assess patient needs and develop individualized plans of care and discharge goals
- Deliver therapeutic interventions and monitor patient progress
- Educate patients and caregivers on treatment plans and safety
- Collaborate with interdisciplinary care teams and healthcare providers
- Maintain accurate and timely documentation
- Supervise Certified Occupational Therapy Assistants as assigned
- Graduate of an accredited Occupational Therapy program
- Current NH OT license
- Minimum of one year of occupational therapy experience
